You can head here to fill out your paperwork. It will need to be completed on a desktop or laptop computer as it is not compatible with mobile or tablet devices, and will submit directly to our secure server once you are finished. If you cannot complete this ahead of time, please show up 15 minutes early to your appointment.
Consultation and History
The doctor will review your paperwork and discuss your health-related problems and concerns, as well as potential treatment options.
The need for x-rays will be decided by your doctor. We usually refer out to Advanced Radiology. They accept most insurances, but cash pricing is also available.
Once finished with the examination and all appropriate studies, patients will receive their first treatment during this same visit, unless there are contraindications found during the initial examination. This may include spinal adjustments, physical therapies, and soft tissue therapies.
We collect payment from patients after each visit. The New Patient appointment cost is $150, and regular appointments are $55. If you have x-rays to review, there will be an additional $40 fee and a longer appointment time.
We do not participate with any insurance company. If you have Out of Network Benefits for Chiropractic under your insurance plan, we can provide you with a Superbill to turn in for reimbursement by your insurance company. If doctors' reports, narratives, or re-coding is neccessary for you to receive benefits, additional fees may be charged. These will vary based on what is requested. If your family is under care, we do have a family plan option.
Dr. Jeremy and Dr. James will give you a plan of care that they will review with you at your second appointment. It is important to note that your plan of care will be based on x-rays, clinical history, and many other factors determined by your doctor, not based on symptoms, like pain.
The goal is always to remove stress from your nervous system to achieve optimum function. Pain relief, better sleep, improved focus, and many many other factors improving are just a side effect.
Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.
